As discussed, please create the OMR for the KYC questionnaire.
I have uploaded 3 files here.

Please include the following details:
Overall heading for all the three forms:
KYC - Know Your Child 2022-23
Let us try to get student level data or else we shall include section and roll number
Headings for the three forms
Students' Self-assessment
Teachers' questionnaire
Include responses for all the three forms in the same OMR
Parents' questionnaire

PFA OMR sheets, If i get the data from Raj sir we can add the students data which require minimum 10 hours.
